非小细胞肺癌中CD44和nm23-H1的表达及临床意义

摘    要:
目的:探讨CD44和nm23-H1蛋白在非小细胞肺癌中的表达及与患者肿瘤临床病理特征之间的关系。方法:应用免疫组织化学法检测42例肺癌组织、21例肺癌癌旁组织、14例肺良性病变组织中CD44和nm23-H1蛋白的表达。结果:肺癌组织中CD44蛋白表达阳性率明显高于癌旁组织及肺良性病变组织,肺良性病变组织中nm23-H1蛋白表达阳性率明显高于肺癌组织(P均<0.01);CD44蛋白的表达与肺癌淋巴结转移及临床分期呈正相关,有淋巴结转移者肺癌组织CD44蛋白表达阳性率明显高于无淋巴结转移者(χ2=4.921, P=0.027);nm23-H1蛋白表达与肺癌淋巴结转移及临床分期呈负相关,有淋巴结转移者肺癌组织的nm23-H1蛋白表达阳性率低于无淋巴结转移者(χ2=4.805, P=0.028)。两者表达阳性率均与肿瘤分化程度无明显相关性。结论:CD44和nm23-H1蛋白在肺癌的发生、发展及转移过程中可能起着协同、调控作用,有可能成为临床上判断和评价预后的重要指标。

Abstract:
Objective: To study the expressions of CD44 and nm23-H1 proteins and their clinical significance in non small cell lung cancer. Methods: Immunohistochemical technique was used to examine the expressions of CD44 and nm23-H1 proteins in 42 non small cell lung cancer, 21 surrounding normal lung tissues and 12 tissues of benign lung disease. ResultsThe expressions of CD44 proteins in non small cell lung cancer were much higher than that in benign lung disease and  the expressions of the nm23 H1 were markedly lower in the same cases(both P<0.01). The expressions of CD44 proteins in lung cancer tissues with metastatic lymph node was significantly higher than that in lung cancer without metastatic lymph node(χ2=4.921, P=0.027), but nm23-H1 protein expression was lower(χ2=4.805, P=0.028).Therefore, the expression of CD44 protein was positively correlated with the metastasis,clinical staging.While the expression of nm23 H1protein was negatively correlated with lymph node metastasis and clinical staging of lung cancer. Neither was correlated with tumor differential degree. ConclusionsCD44 and nm23-H1 gene proteins might play an important coordinated regular role in the carcinogenesis,development and metastasis of lung cancer and would be the key biological marks in judging and evaluating the prognosis of non-small cell lung cancer.
