| 摘 要： | 目的：探讨肥胖儿童非酒精性脂肪肝炎（NASH）与胰岛素、瘦素抵抗的关系。方法：对符合NASH诊断的23例患儿,符合单纯性肥胖诊断的17例儿童及20例正常儿童分别检测血谷丙转氨酶、游离脂肪酸、甘油三酯、胰岛素、瘦素,比较它们的水平并分析谷丙转氨酶与各指标的关系。结果：NASH组患儿血游离脂肪酸、甘油三酯、胰岛素、瘦素水平高于单纯肥胖组和对照组（P〈0.05）。相关分析显示谷丙转氨酶与游离脂肪酸、甘油三酯、胰岛素、瘦素和HOMA-R各指标间存在正相关（r=0.407,r=0.32,r=0.338,r=0.384,r=0.524;P〈0.01,P〈0.05,P〈0.05,P〈0.05,P〈0.01）。结论：NASH患儿存在着血脂升高、高胰岛素和高瘦素血症,瘦素抵抗与胰岛素抵抗单独或共同构成NASH的发病因素之一。
