肺叶切除支气管袖状成形术治疗肺癌围手术期的处理

目的：探讨肺叶切除支气管袖式成形术治疗肺癌的围手术期处理及并发症的防治。方法：回顾性总结31例中心型肺癌肺叶切除支气管袖式成形术的围手术期处理的临床经验。结果：31例患者中,2例出现支气管胸膜瘘,心功能不全3例,不同程度心率失常21例,肺部感染15例,胸腔中-大积液5例,肺不张8例,经治疗均顺利渡过围手术期。结论：肺叶切除支气管袖式成形术较为复杂,并发症较多,恰当的围手术期处理,更好防治术后并发症,使绝大多数患者安全度过围手术期。

Perioperative Management of Lobectomy with Bronchial Sleeve Resection and Plasty in the Treatment of Primary Lung Cancer

Objective： To study the perioperative management and prevention for complication after lobeetomy with bronchial sleeve resection and plasty in the treatment of center lung cancer. Method： Clinical experience of 31 center lung cancer with perioperatively treatment was reviewed. Result： In 31patients, bronchopleural fistola occured in 2 patients,heart failure in 3, cardiac dysrythmia in 21, pulmonary infection in 15 ,initialingheavy pleural effusion in 5, ateleetasis in 8. Conclusion： Lobectomy with bronchial sleeve resection and plasty complicated with more complications. Proper perioperation management can help patients to prevent complication and pass the perioperative period.
