腹腔镜与开腹直肠癌根治术的近期临床疗效比较

目的评价并比较腹腔镜与开腹直肠癌根治术的临床疗效。方法回顾性分析我院收治的82例直肠癌患者的临床资料,按术式随机分为腹腔镜手术组和开腹手术组,观察两组手术保肛率、时间、术中出血量、清扫淋巴结数量、术后恢复下床活动时间、肠道恢复时间、住院时间、并发症、生存率。结果两组患者均手术成功,无瘤生存至今。腹腔镜能方便双吻合器稍提高低位直肠癌的保肛率,手术时间较开腹组长(P<0.05),而术中出血量较少(P<0.05)。腹腔镜术组患者术后肠道恢复时间,下床活动时间及住院时间均较开腹手术组缩短(P<0.05)。腹腔镜手术组并发症发生率显著低于开腹组(P<0.05)。结论腹腔镜直肠癌根治术保肛率稍高、安全、可行、有效,优于开腹手术。

The Comparison of Short-Term Clinical Efficacy between Laparoscopic and Laparotomy Radical Resection for Rectal Cancer

Objective To evaluate and compare the clinical efficacy of laparoscopic and laparotomy radical resection for rectal cancer.Methods The clinical data of 82 cases with rectal cancer in our hospital were analyzed retrospectively,and randomly divided into laparoscopic and laparotomy radical resection group.The operative time,the amount of intraoperative bleeding,the lymphadenectomy,the postoperative recovery time of out-of-bed activities,the bowl function recovery,the time of hospitalization,complications,and survival rate were observed.Results 2 groups of operation are all successfull and all patients are alive with tumor-free till now.laparoscopy facilitate double stapling technique to increase easily the rates of anus preservation slightly in low rectal cancer,The operative time was longer than that of laparotomy group,and the amount of intraoperation bleeding was lower(P<0.05).The recovery time of bowel function,the recovery time of out-of-bed activities,and the time of hospitalization decreased signifi cantly(P<0.05).The complications of laparoscopic group were signifi cantly lower than the laparotomy group(P<0.05).Conclusions Laparoscopic radical resection increase the rate of anus preservation slightly,it was safe,feasible,effective,and better than laparotomy for rectal cancer.
