头孢唑啉钠对抗癫痫药物浓度的影响

目的 :探讨头孢唑啉钠 (CEF)对卡马西平 (CBZ)血药浓度的影响。方法 :选取年龄、体重、服用CBZ剂量均相匹配的癫痫患儿 31例 ,随即分为两组 :观察组加用CEF ,对照组单用CBZ。用高效液相色谱法测定患儿体内血清CBZ浓度。结果 :①观察组患儿血清CBZ浓度明显高于对照组 (P =0 .0 0 1) ;②服用CBZ患儿合用CEF前后其CBZ浓度有统计学意义 (P =0 .0 0 0 1) ,用CEF治疗后CBZ浓度显著增高。结论 :CEF可使CBZ浓度明显升高 ,癫痫患儿联合应用CEF时 ,应进行CBZ血药浓度检测 ,以防出现中毒反应。

Effect of cefazolin on carbamazapine serum concentration in children with epilepsy

Objective: To investigate the effect of cefazolin on carbamazepine(CBZ) serum concentration in children with epilepsy. Methods: The minimal steady state concentrations of CBZ from children with epilepsy were measured using high performance liquid chromatography (HPLC). Results: The serum concentrations of CBZ before and after coadministrating cefazolin were quantitatively very different (P=0.0001). Much higher concentrations of CBZ were also found in children who had taken cefazolin than those in children who had taken only CBZ (P=0.001). Conclusion: There is apparent drug interaction between CBZ and cefazolin. If possible, cefazolin should not be used in patients taking CBZ. If judgement suggests clinically that cefazolin should be used, the serum drug levels should be monitored closely, and the patient should be informed of the signs and symptoms of CBZ toxicity.
