氢化可的松琥珀酸钠治疗重症哮喘的疗效观察

目的探讨氢化可的松琥珀酸钠对重症哮喘患者的疗效。方法选取2010年1月。2012年3月收治的重症哮喘患者56例，随机分为对照组和观察组各28例，对照组在常规治疗的基础上，加用地塞米松20mgQD静脉滴注，观察组在常规治疗的基础上，加用氢化可的松琥珀酸钠200mgQ8h静脉滴注，所有患者根据病情需要，连续用药4。6d，对比分析地塞米松与氢化可的松琥珀酸钠对重症哮喘患者的有效率、肺通气功能、血气分析等指标。结果对照组患者的有效率为57．14％，观察组患者的有效率为78．57％，两组之间的差异有统计学意义（P〈0．05）；对照组和观察组患者经治疗后肺通气功能均明显改善，但观察组患者肺通气功能改善情况较对照组更高，两组之间的差异有统计学意义（P〈0．05）；对照组与观察组治疗前后的血气分析各指标经统计学分析，差异有统计学意义（R0．05）。结论氢化可的松琥珀酸钠治疗重症哮喘患者缓解率高，适合在临床广泛应用。

The efficacy of Hydrocortisone in patients with severe bronchial asthma

Objective BTo observe the curative effect of hydrocortisone in the treatment of severe bronchial asthma. Methods 56 patients were enrolled in this study during January 2010 to March 2012. All the patients were randomized,volunteered to divide into two groups,which include 28 patients.The control group was treated with Dexamethasone （20mg QD intravenous） and the experimental group was treated with hydrocor- tisone （200mg Q8h intravenous）. Both of the two groups were treated 4-6 days. Then we followed up in long term for the curative effect, lung function index and blood gas analysis. Results The control group＇s total ef- fective rate is 57.14%（ 16/28 ）, the experimental group ＂s total effective rate is 78.57%（22/28 ）, it had signifi- cant effect （P〈0.05）. The experimental group ＂s lung function index （FEV1 and FVC ） were significantly bet- ter than those of the control group. The pH,PaO2,PaCO2 after treatment is better than the control group,it had significant effect （P〈0.05）. Conclusions Hydrocortisone treatment in patients with Severe bronchial asthma have a higher effective rate. It worth more applications in clinical practice.
