儿童肺炎支原体肺炎136例临床分析

目的探讨儿童肺炎支原体肺炎(MPP)的临床诊治水平。方法对我院2009年1月～2010年10月住院治疗的136例MPP患儿临床资料进行回顾分析。结果本组患儿以6个月～3岁的婴幼儿为主。临床表现为发热,持续性咳嗽,喘息,肺部体征不明显,X线胸片变化较多。多数患儿外周血白细胞总数正常。肺外并发症可见多系统损害。血清肺炎支原体特异性IgM抗体(MP-IgM)均阳性。所有病例用红霉素或阿奇霉素治疗3～5w均治愈,部分严重病例应综合个体化治疗。结论儿童MPP发病年龄有提前趋势,症状体征多变,无特征性,肺外并发症可累及多系统,红霉素、阿奇霉素是有效的治疗药物,严重病例综合治疗。

CHILDREN MYCOPLASMA PNEUMONIA CLINICAL ANALYSIS OF 136 CASES

Objective To probe into the clinical diagnosis and therapy of Mycoplasma pneumoniae pneumonia（MPP） in children。Methods 136 hospitalized patients with MPP in our hospital in January 2009-October 2010 were retrospectively analyzed.Results This group of children is mainly 6 months to 3 years old.Clinical manifestations were fever,persistent cough,wheezing,lung obvious signs,X-ray is much more varied.The majority of peripheral blood leukocyte count is normal.Extrapulmonary complications can be multi-system damage.Mycoplasma pneumoniae-specific IgM serum antibodies（MP-IgM） were positive.All patients treated with erythromycin or azithromycin were cured in 3 to 5 weeks,and some serious cases required comprehensive individualized treatment.Conclusion The onset age of children MPP trends to be earlier,signs and symptoms varied,no characteristic,pulmonary complications can affect multiple systems,erythromycin,azithromycin is effective treatment,comprehensive treatment of severe cases needed.
