超声引导下经皮经肝胆囊穿刺术／PTGBA在老年急性胆囊炎中的应用

目的探讨超声引导下经皮经肝胆囊穿刺术（PTGBA）应用于老年急性胆囊炎的安全性和实用性。方法回顾分析2002年1月-2007年12月期间接受PTGBA治疗的67例老年急性胆囊炎患者的临床资料。结果67例患者均穿刺成功，其中48例经1次操作6—24小时后症状缓解，另19例患者经重复穿刺抽吸后症状缓解。2例患者在症状缓解后3个月和5个月胆囊炎复发，后再次经穿刺术后缓解。16例患者在PTGBA后半年内行胆囊切除术。所有患者均无严重并发症发生。结论在老年急性胆囊炎治疗中，PTGBA是一种安全、简单、有效的操作方式，且有可重复、易操作的优点。

Percutaneous transhepatic gallbladder aspirationforacute cholecystitis in elderly patients by ultrasound-guided

Objective The aim of this study was to evaluate the safety and usefulness of percutaneous transhe- patic gallbladder aspiration （PTGBA） for the treatment of acute cholecystitis. Methods The clinical data of 67 pa- tients who underwent PTGBAbetween January2002 and December 2007 were analyzed retrospectively. Results PTG BAwas successfully performed in all patients, repeated. Single PTGBA achieved improvement in 48 of 67 patients after 24 N48h. In 19 of the remaining patients,the second PTGBA was effective. Twopatients whose conditions im- proved with a single PTGBA experienced a recurrence at 3 and 5 months,respectively, after PTGBA. Andachieved improvementby PTGBA. 16 accepted cholecystectomy in half a year after PTGBA. No other severe complications re- lated to PTGBA were observed in any patients. Conclusion For the treatment of acute cholecystitis that does not re- act to conservative therapies,PTGBA is a safe, simple, and effective treatment modality that can be performed repet- itive and easily.
