精神分裂症立体定向手术后多巴胺代谢变化及疗效观察

摘    要:目的探讨立体定向手术对精神分裂症患者多巴胺(DA)代谢的影响,并观察其治疗效果。方法立体定向手术治疗精神分裂症;高效液相色谱-电化学法(HPLC-ECD)测定手术前后患者CSF中DA及高香草酸(HVA)水平;简明精神病量表(BPRS)和阴性与阳性症状量表(PANSS)评定手术疗效。结果精神分裂症患者手术前CSF中DA、HVA水平显著高于对照组(P<0.05);手术后DA、HVA水平显著下降(P<0.05);手术显效率为60%(15/25),有效率为100%(25/25)。结论立体定向手术具有明显的DA能神经阻滞作用,是精神分裂症的一种疗效明显的治疗方法。

Abstract:Objective To study the effect of stereotactic operation on the CSF levels of dopamine(DA)and homovanillic acid(HVA)in patients with intractable schizophrenia.Methods The levels of DA and HVA in the CSF were detected by high performance liquid chrimatography and electrochemical detector.The therapeutic efficacy was estimated by BPRS and PANSS.Results The CSF levels of DA and HVA were significantly higher(P<0.05)in the patients with schizophrenia before the operation than that in the nomal control,and decreaseed significantly(P<0.05)after the operation as compared with before the operation.The markedly effective ratio of the operation was60%(15/25)and the total effective ratio was100%(25/25).Conclusion The stereotactic operation can block dopaminergic nerve and is an effective treatment for schizophrenia.
