胃间质瘤多层螺旋CT双期扫描的诊断价值

目的探讨多排螺旋CT对胃间质瘤的诊断价值。方法回顾性分析经手术及病理免疫组织化学证实的25例胃间质瘤的CT平扫和增强表现。结果肿瘤均单发,呈均匀或不均匀软组织密度,其内可见出血、坏死、囊变及钙化。腔内型4例,腔外型14例,哑铃型7例。良性19例,直径均〈5 cm,密度均匀,强化较均匀;恶性6例,直径均〉5 cm,密度多不均匀,见囊变、坏死,且强化不均匀。静脉期病变强化程度高于动脉期。瘤内见点状钙化灶1例。结论多排螺旋CT可同时观察肿瘤本身、周围脏器及有无转移等重要征象,对鉴别良、恶性肿瘤,临床早期诊断和治疗以及评估预后有重要价值。

Value of muilti-spiral CT scan in diagnosis of gastric stromal tumors

Objective To examine the diagnostic value of multi-spiral CT（MSCT） for gastric stromal tumors（GST）.Methods Twenty-five patients with GST confirmed by pathology and immunohistochemistry were included in the study.Both plain and enhanced CT findings were retrospectively analyzed.Results All the tumors were solitary lesion presenting homogeneous or inhomogeneous density with necrosis,cystis formation and calcification.The types of lesions were intraluminal（n=4）,exophytic pattern（n=14） or dumbbell pattern（n=7）.Nineteen benign cases with diameter was〈5 cm,presented uniform density and homogenous enhancement;6 maglinant cases with diameter〉5 cm presented uneven density,heterogenous enhancement,necrosis and cystic degeneration.The extend of enhancement in venous phase was higher than that of arterial phase.One case showed intratumor punctuate calcification.Conclusion Multi-spiral CT is of value for early diagnosis of GST.
