新发传染病病毒性脑炎及出血热的鉴别诊断及临床诊断辅助识别系统研究

摘    要:目的建立新发传染病病毒性脑炎及出血热的鉴别诊断及临床诊断辅助识别系统。方法以出血热、脑炎为主要表现的病毒性EID为主线,系统研究其临床特征,通过先进的信息化技术,建立其临床诊断和鉴别诊断辅助识别系统。结果确定了一套稳定和准确的临床诊断指标体系;建立了专家辅助诊断系统的评价体系并建立了贝叶斯网络。对37种感染性疾病,154份病例应用该系统进行初步验证。结果显示临床诊断与第一诊断符合率59.7%;前三位提示率77.9%;前五位提示率85.4%。结论新发传染病病毒性脑炎及出血热的鉴别诊断及临床诊断辅助识别系统符合临床思维,具有提示、引导、指导和培训功能,并具有较好的准确性、兼容性、稳定性、灵活性和实用性。

Abstract:Objective To establish the computer identification-aided system for differential diagnosis and clinical diagnosis of emerging infectious diseases(EID),especially for viral encephalitis and hemorrhagic fever.Methods Setting viral EID displayed from hemorrhagic fever and encephalitis as the main line,this study explored their clinical features systematically;building up the identification-aided system for their differential diagnosis and clinical diagnosis with the help of advanced information technology.Results A set of stable and accurate index system was established for clinical diagnosis.The evaluation system of experts-aided diagnostic system was built up and the Bayes network was set up.The system was applied to 154 cases which referred to 37 kinds of contagious diseases for preliminary verification.And the results showed that the coincidence rate of clinical diagnosis and first diagnosis of this system was 59.7%;and the suggestion rate of first three diagnoses was 77.9%,that of first five diagnoses was 85.4%.Conclusions The identification-aided system for differential diagnosis and clinical diagnosis of emerging infectious diseases especially for viral encephalitis and hemorrhagic fever is in accordance with the clinical thought;it presents the functions of suggestion,guidance,instruction and training,and shows accuracy,compatibility,stability,flexibility and practicability.
