85例极低出生体重早产儿的生长速率和影响因素分析

目的分析极低出生体重早产儿的生长速率和影响因素。方法本次研究中的极低出生体重早产儿共85例，按照婴儿是否为适于胎龄将其分为两组，适于胎龄（AGA）组40例，小于胎龄（SGA）组45例，按照生长速率重新分为两组，生长速率较快组，38例患儿的平均生长速率≥15g／（kg·d），生长速率较慢组，47例患儿的平均生长速率〈15g／（kg·d）。对结果进行统计。结果AGA组患儿胎龄、体重增长速率明显低于SGA组，出生体重明显高于SGA组，差异有统计学意义（P〈0．05）；生长速率较快组患儿出生体重、开始喂养日龄均明显低于生长速率较慢组，差异有统计学意义（P〈0．05）。结论在住院期间大部分极低出生体重早产儿的生长速率与正常胎儿还是有一定的差距，应保证足够的热量、多种氨基酸摄入量及液体入量，给予积极合理肠内肠外营养。

Analysis of growth rate and affecting factors in 85 cases with very low birth weight preterm children

Objective To analyze the growth rate and affecting factors in very low birth weight preterm children. Methods 85 cases with very low birth weight preterm children in the study were divided into two groups according to whether be appropriate for gestational age children, will be appropriate for gestational age（AGA） group in 40 cases, small for gestational age（SGA） group in 45 cases, and according to growth rate divided into two groups, growth rate faster group with the average growth rate of 38 cases ≥ 15g/（kg·d）, growth rate slower group with the average growth of 47 cases 〈 15g/（kg · d）. The results were statistically analyzed. Results Children with AGA group with gestational age and growth rate of body weight were significantly lower than SGA group, birth weight was significantly higher than SGA group, the difference was statistically significant（P 〈 0.05）; growth rate faster group with birth weight and start feeding days were significantly lower than growth rate slower group, the difference was statistically significant （P 〈 0.05）. Conclusion The growth rate of most very low birth weight preterm children in the hospital is still a certain mismatch with normal fetal growth rate, so sufficient heat, amino acids intake and fluid intake should be ensured, positive rational enteral parenteral nutrition should be given.
