前列腺癌的MRI动态增强特征与微血管密度和前列腺特异性抗原的相关性的初步研究

摘    要:目的评价前列腺癌的动态增强特征与微血管密度(MVD)和前列腺特异性抗原(PSA)的相关性,为前列腺癌的诊断提供帮助. 资料与方法对32例前列腺癌患者进行MRI动态增强扫描,设立40例良性前列腺增生(BPH)患者进行对照,观察病灶的动态增强MRI的强化参数:开始时间、最大信号强度及早期强化率,获得感兴趣区的病理标本,进行免疫组织化学MVD的测定,比较分析前列腺癌和BPH的动态增强参数、两者的MVD和PSA的相关关系. 结果前列腺癌的MRI动态增强的强化参数开始时间、最大信号强度及早期强化率与BPH不同(P分别<0.01、0.05、0.01).前列腺癌的MVD的测定水平高于BPH(P<0.001).前列腺癌和BPH的MRI动态增强参数中的最大信号强度和早期强化率与MVD有相关性(P<0.05或0.01).前列腺癌的血清PSA与MVD的水平具有相关性(P<0.05). 结论前列腺癌MRI动态增强特征、前列腺癌的MVD与PSA的水平具有一定的相关性,MRI动态增强与PSA的测定相结合能为前列腺癌的诊断提供帮助.

Abstract:Objective To evaluate the correlations about the characters of dynamic contrast-enhanced MRI,microvessel density(MVD) of the prostatic cancer and the prostate-specific antigen(PSA) and provide information for the diagnosis of the prostatic cancer.Materials and Methods 32 patients with prostatic cancer histopathologically verified were underwent dynamic contrast-enhanced RMI.40 patients proved to be benign prostatic hyperplasia(BPH) were performed with dynamic contrast-enhanced MRI as control group.The results of dynamic contrast-enhanced MRI were evaluated by enhancement parameters including onset time, maximum signal intensity and early-phase enhancement rate.The pathologic specimens of region of interest(ROI) were obtained,in which immuno-histochemical MVD measurements were performed.The relationships among dynamic contrast-enhanced parameters,the level of MVD and the level of PSA of the prostatic cancer and BPH were analyzed.Results The enhancement parameters of dynamic contrast-enhanced MRI including onset time,maximum signal intensity and early-phase enhancement rate were differed from those of BPH((P<)0.05) or 0.01.The level of MVD of prostatic cancer were significantly higher than those of BPH(P<0.001).The maximum signal intensity and early-phase enhancement rate in both prostatic cancer and BPH were revealed associations with the level of MVD(P<0.05) or 0.01.The level of PSA of the prostatic cancer was revealed association with the level of MVD(P<0.05).Conclusion There are correlations between enhancement parameters of dynamic contrast-enhanced MRI,MVD of prostatic cancer and PSA,dynamic contrast-enhanced MRI combined with PSA are helpful to the diagnosis of prostatic cancer.
