自膨式支架治疗症状性颅内血管狭窄的并发症

目的 分析应用Gateway-Wingspan支架系统治疗症状性颅内血管狭窄的并发症,探讨其安全性.方法 回顾性分析应用Wingspan治疗31例症状性颅内血管狭窄患者的资料,研究发生并发症的原因.结果 31例患者中4例出现并发症,并发症发生率为12.9%,全部为缺血性并发症,3例为病变相关性穿支梗死,1例为非病变相关性,术中无急性血栓形成.2例患者具有一过性神经功能障碍,2例患者具有持久神经功能异常.没有患者死亡.结论 应用Wingspan支架治疗颅内血管狭窄比较安全;本组病例显示该系统引起穿支血管梗死的风险相对较高,但需进一步临床实践检验.

Complications of stenting with Gateway-Wingspan system for symptomatic intracranial atherosclerotic stenosis

Objective To investigate the safety of treatment of symptomatic intracranial atherosclerotic stenosis with Gateway-Wingspan system by analyzing cases with complications.Method 31 patients with symptomatic intracranial atherosclerotic stenosis treated with angioplasty with Wingspan system were respectively analyzed and the emphasis was paid to the cases with complications.The data was collected such as characteristic of patients and lesions,selection of balloon and stent and radiographic informations.Resuits The complications were found in 4 patients(12.9%),three of which were lesion-related infarction of perforation artery and one was non-lesion-related infarction.There were two cases of transient neurologic dysfunction,one case of defect of visual field and one case of hemiplegia.The morbidity and serious morbidity were respectively 6.4%and 3.2%and no death occurred.There was no hemorrhagic event.Conclusions Angioplasty with Wingspan system is a safe method for the treatment of symptomatic intracranial atherosclerotic stenosis.The complication rate is acceptable.The Wingspan system may increase the risk of perforating artery infarction,which needs more practice to verify.
