| 摘 要： | 目的观察高眼压状态下抗青光眼手术的疗效.方法对37例(40眼)持续高眼压性青光眼患者,先行前房穿刺缓慢放出房水后,再行小梁切除术,术后观查滤过泡、前房及眼压、视力变化情况.结果术后1月眼压控制≤21 mmHg(1 mmHg=0.133kPa)者31者,占77.5%,需局部用药方可控制者7眼,占17.5%,术后视力提高或保持不变者32例,占80%.术后发生浅前房5例,前房积血3例,脉络膜脱离1例,未见脉络膜上腔出血等严重并发症的发生.结论高眼压状态下前房穿刺后再行小梁切除术,可有效控制眼压,保持或改善视力,是持续高眼压性青光眼治疗的有效术式之一.

| Abstract: | Objective To observe the effects of trabeculectomy with paracentesis under the condition of high intraocular pressure(IOP).Methoeds Paracentesis was taken to reduce IOP gradually, then trabeculectomy was taken for 37 cases (40 eyes). Filter bleb,IOP,visial acuity and anterior chamber condition were observed postoperatively. Results Postoperative cases with IOP≤21 mmHg count 77.5%(31 eyes). Visual acuity was improved or kept the same in 32 eyes (counts 80%). Postoperative shallow anterior chamber,hyphema and choroidal detachment were found in some cases but soon cured with madication. No serious complications were found. Conclusion Trabeculectomy with paracentesis can safely reduce IOP and keeps an acceptable visual acuity, so is an affective method for glaucoma with persistent high IOP. |
