彩色多普勒在脑梗塞病中的临床应用

目的：探讨脑梗塞病人与正常人颈动脉、椎动脉之间的各种病变和血流动力学关系。方法：采色多普勒检测35例脑梗塞病人，42例年龄相当正常人颈动脉、椎动脉。结果：梗塞组内径明显增宽，内膜增厚；舒张期流速及平均流速降低；收缩期峰值流速无明显差异；搏动指数、阻力指数增高；管腔内斑块发生率高。结论：彩色多普勒能反映脑梗塞后血管的顺应性、弹性；评价血管收缩，舒张功能和阻力状况，从而在病变早期为临床提供信息。

Clinical application of color doppler in hemispheric infarction

To investigate the relationship of the pathological changes and hemodynamics in common carotid artery, internal carotid artery and vertebral artery between the patients with hemispheric infarction and normal controls. Methods: 35 patients with hemispheric infarction and 42 normal controls were examined with color doppler ultrasound. Result: In the groulp of hemispheric infarction, the vascular lumen become wide, and intemal membrane become thick as compared with controls. The Evd and Vmean decreased, PI and RI increased, the places in vascular lumen formed more commoney, the Spv has no significant difference between two groups. Conclution: Color doppler ultrasound observation can reflect the changes of vascular compliance and elasticity. It also can evaluate vascular constraction capability and the condition of resistance. It might provide earlier information of hemispheric infarction for clinical doctors.
