中西医结合治疗恶性心包积液19例

[目的]探讨治疗肿瘤患者晚期合并恶性心包积液的有效方法。[方法]19例恶性心包积液患者,均CVCC管心包腔留置,待引净积液后,经导管向心包腔内注射乌体林斯或香菇多糖,若每天引流量<100mL,或经超声心动图检查,证实仅有少量或无积液后,可拔除导管,在引流期间同时服用以苓桂术甘汤为主方辨证加减的汤药。[结果]完全缓解9例,部分缓解7例,共计16例有效,总有效率为84.21%,所有患者在治疗后胸闷、气短等症状明显改善,无明显并发症出现。随访19例患者,其中有14例生存。[结论]中西医结合治疗恶性心包积液不仅对于身体状况良好的患者,而且对于体质虚弱、肿瘤晚期出现恶病质、临床症状复杂多样的患者(尤其是老人)来说,是一种较理想的治疗方法。是临床开展治疗恶性心包积液另一有效的途径。

Traditional chinese medicine combined with western medicine for 19 malignant hydropercardium

[Objective] To evaluate the effective treatment of advanced tumor combine with malignant hydropercardium patients.[Methods]Gave CVCC ducts indwell in pericardial cavity,after drained all hydrops,injected Utilin"S"or Lentinan into pericardial cavity through ducts.If the volume less than 100mL,or the echocardiogram proved only a little or no hydrops,pulled out the ducts.Took the decoction based on Poria,Cinnamon,Bighead Atractylodes and Licorice Decoction at the same time.[Results]CR 9 cases,PR 7 cases,total effective rate was 84.21%.The symptoms such as chest distress and short breath all had improved and no obvious complication.Follow-up survey showed that 14 patients survival in 19 cases.[Conclusions] TCM combined with western medicine treat malignant hydropercardium not only suit for well constitution patients,but also an ideal treatment for poor constitution,advanced tumor combine with cachexia,complicated symptoms patients.It's another effective treatment for malignant hydropercardium.
