补肾降浊汤治疗慢性肾小球肾炎临床研究

目的:观察补肾降浊汤对慢性肾小球肾炎患者肾功能、免疫功能的影响。方法:将慢性肾小球肾炎患者98例,按随机数字表法分为观察组与对照组各49例。对照组予以常规西医治疗,观察组在对照组的基础上联合补肾降浊汤治疗,2组疗程均为8周。比较2组临床疗效,中医证候总积分、肾功能相关指标(血清尿素氮、血肌酐、24 h尿蛋白定量、1 h尿红细胞排泄率)、免疫功能相关指标CD4+、CD8+、CD4+/CD8+、免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、补体C3]水平变化。结果:观察组总有效率为91.84%,对照组为75.51%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组中医证候总积分均较治疗前明显降低(P<0.05),且观察组低于对照组(P<0.05)。治疗后,2组血清尿素氮、血肌酐、24 h尿蛋白定量、1 h尿红细胞排泄率均较治疗前降低,且观察组各项指标均低于对照组(P<0.05)。治疗后,2组CD8+、IgA水平均较治疗前降低(P<0.05),CD4+/CD8+、IgM、补体C3水平均较治疗前升高(P<0.05);且观察组上述指标改善较对照组更显著(P<0.05)。结论:补肾降浊汤治疗慢性肾小球肾炎疗效确切,可有效缓解症状,改善患者肾功能与免疫功能。

Clinical Study on Bushen Jiangzhuo Tang for Chronic Glomerulonephritis

Objective:To observe the effect of Bushen Jiangzhuo tang on kidney function and immune function in patients with chronic glomerulonephritis.Methods:A total of 98 cases of patients with chronic glomerulonephritis were divided into the control group and the observation group according to the random number table method,49 cases in each group.The control group was treated with routine western medicine,and the observation group was additionally treated with Bushen Jiangzhuo tang based on the treatment of the control group.Both groups were treated for eight weeks.The changes in clinical effect,total Chinese medicine syndrome scores,related indexes of kidney function including serum urea nitrogen,serum creatinine,24-hour urinary protein,1-hour urine red blood cell excreting rate,and related indexes of immune function including CD4+,CD8+and CD4+/CD8+,immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM)and complement C3 in the two groups were compared.Results:The total effective rate was 91.84%in the observation group and 75.51%in the control group,the difference being significant(P<0.05).After treatment,the total Chinese medicine syndrome scores in the two groups were significantly decreased when compared with those before treatment(P<0.05),and the score in the observation group was lower than that in the control group(P<0.05).After treatment,serum urea nitrogen,serum creatinine,24-hour urinary protein,1-hour urine red blood cell excreting rate in the two groups were decreased when compared with those before treatment,and the indexes in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of CD8+and IgA in the two groups were decreased when compared with those before treatment,and the levels of CD4+/CD8+,IgMand complementC3 were increased(P<0.05);the improvement of the above indexes in the observation group was more significant than that in the control group(P<0.05).Conclusion:Bushen Jiangzhuo tang has definite curative effect in treating chronic glomerulonephritis,which can effectively relieve symptoms and improve kidney function and immune function of patients.
