妊娠期糖尿病合并子痫前期29例临床分析

目的:探讨妊娠期糖尿病(GDM)合并子痫前期(pre-ec lampsia)的处理及妊娠结局。方法:回顾性分析2002年1月~2004年7月我院诊治的妊娠期糖尿病合并子痫前期29例患者,将其按子痫前期的严重程度分为轻度组(Ⅰ组)与重度组(Ⅱ组),随机抽取同期分娩无其它合并症的轻度子痫前期(Ⅲ组)与重度子痫前期各20例(Ⅳ组),观察各组的特点及妊娠结局。结果:GDM并pre-ec lampsia的羊水粪染、新生儿窒息、新生儿转入重症监护室、低体重儿的发生率分别为6.89%、0%、10.34%、27.59%,与单纯pre-ec lampsia比较,无显著差异。早产、产后出血的发生率分别为34.5%、44.8%,单纯pre-ec lampsia患者两者发生率分别为22.5%、17.5%,有显著差异。结论:对GDM合并pre-ec lampsia早期诊断、积极处理有助于改善围生儿预后,提高产科质量。

Clinical Study of 29 cases of Gestational Diabetes Mellitus combined with Pre -eclampsia

Objective:To investigate the treatment and pregnant outcomes of gestational diabetes mellitus(GDM) combined with pre-eclampsia.Methods:29 cases of GDM associated pre-eclampsia were collected retrospectively.Among them,10 cases were mild PIH(group Ⅰ),19 cases were severe pre-eclampsia(group Ⅱ).The other 40 cases of pre-eclampsia were divided into two groups,mild pre-eclampsia(groupⅢ)and severe pre-eclampsia(group Ⅳ).Results:In GDM and pre-eclampsia group,the incidences of aminiotic fluid meconium staining,neonatal asphyxia,neonatal intensive care unit admission and low birth weight were 6.89%,0%,10.34%,27.59% respectively,there were no significant differences compared to the pre-eclampsia Group.But the incidences of preterm birth(34.5%)and postpartum hemorrhage(44.8%) were higher than the pre-eclampsia group.Conclusion:Early diagnosis and timely treatment can improve the perinal outcomes of GDM and pre-eclampsia.
