胎盘部位结节

目的 报告6例胎盘部位结节的临床及病理学特征,进一步认识到该病变是一种残留的胎盘部位组织,与当前妊娠缺乏联系。方法 在刮宫及子宫切除标本常规HE切片的光镜观察中偶然发现。其中2例分别做角蛋白、胎盘促乳素及绒毛膜促性腺激素三项免疫组化标记。结果 光镜下为界清的广泛玻璃样变的结节,病变细胞为中间滋养层细胞,其免疫组化标记角蛋白为弥温强阳性,胎盘促乳素少数细胞强阳性。结论 胎盘部位结节是良性病变,须与正常宫内妊娠、胎盘部位滋养层细胞肿瘤及一种宫项鳞癌相鉴别。

Placenta site nodules and plaques

Objective The clinical and pathological features of 6 cases of placental site nodule/plaques(PSN/P) are described.The lesions represented a remnant of placental site tissue,which had tight association with ancient pregnancy.Methods They were incidental finding in curettage or hysterectomy specimens and microscopic findings only with HE stained.Two of the 6 cases were stained immunohistochemically for cytokeration AE1/AE3(CKAE1/AE3),human placental lactogen(hPL),and human chorionic gonadotropin (hCG).Results Microscopic examination disclosed mostly well circumscribed and extensively hyalinized nodules with typically intermediate trophoblast cells (Its),in which staining for CKAE1/AE3 was diffusedly positive and hPL was focally positive.Conclusion PSN/P is benign in nature and it must be differentiated from pregnancy in the uterus,the placental site trophoblastic tumor and a kind of squamous cell carcinoma of cervix.
