保留腹腔自主神经丛的直肠癌根治术(附15例临床分析)

目的　探讨直肠癌保留盆腔自主神经丛根治术对患者性功能、排尿功能及根治效果的影响。方法　分别对 15例男性直肠癌患者行保留盆腔植物神经根治术、常规根治术及扩大根治术 ,观察患者阴茎勃起功能、射精功能及排尿功能、局部复发情况。结果　保留盆腔自主神经术 15例中 ,1例勃起障碍 ,3例有射精障碍 ,无 1例排尿功能障碍及肿瘤局部复发 ;常规直肠癌根治术 14例中 ,全部有射精障碍 ,9例有勃起障碍 ,术后排尿困难持续时间明显延长。 2组间相差显著。结论　直肠癌保留盆腔自主神经根治术能有效保护患者术后的性功能、排尿功能 ,且对肿瘤的根治效果无明显影响。

Radical Operation of Rectal Carcinoma with Conservation of Pelvic Autonomic Nerve

Objective To discuss the affection of radical operation of rectal carcinoma with conservation of pelvic autonomic nerve on sexual, urinary function and operation. Methods 29 cases of rectal carcinoma patient were involved and 15 cases among them were treated with radical operation combined conservation of pelvic autonomic nerve and 14 cases were treated with routine radical operation. The erection, ejaculation, urinary function and local recurrence were observed in both groups. Results There was 1 cases erection barrier, 3 cases ejaculation barrier, no urinary barrier and local recurrence in conservation group and there were all ejaculation barrier, 9 cases erection barrier and duration of dysuria prolonged with significantly difference. Conclusion The conservation pelvic radical operation could protect effectively the sexual and urinary function without affection for radical operation.
