应用芬太尼透皮贴剂治疗癌痛临床观察

目的　观察芬太尼透皮贴剂 (多瑞吉 )在中重度癌痛病人中的止痛疗效 ,不良反应及对生活质量的影响。方法　共有 6 2例确诊为恶性肿瘤并伴有中重度癌痛者入组 ,采用自行评分法如疼痛指数在≥ 6时。给予多瑞吉以小剂量 2 5ug/h开始或根据原使用的吗啡剂量折算出多瑞吉剂量 ,贴于前胸腹壁 ,每 3天更换贴剂 ,并酌情调整剂量。在用药前及用药后 15天及 30天分别进行生活质量的评估。结果　用药后止痛有效率为 70 .97%( 4 4/6 2 )。对慢性持续性疾病的止痛疗效优于急性疼痛。不良反应发生率低 ,多为轻度的嗜睡、恶心呕吐等 ,用药后生活质量有不同程度的改善。特别是在食欲、睡眠和精神方面有较大的改善。但仍有 15 %的患者达不到止痛的目的。结论　芬太尼透皮贴剂对于伴有中重度癌痛的多数患者止痛疗效满意 ,不良反应发生率低 ,能改善患者的生存质量 ,适于多数中重度癌痛者无创止痛无痛死亡的要求。是胃肠外阿片类镇痛的良好替代品。

Observation on fentany transdermal system in the treatment of cancer pain

Objective To observe the efficacy and side effect of Fentany transdermal system (Durogesic) in the treatment severe cancer pain.Methods 62 patients who were enrolled into test were confirmed to be malignant carcinomas.According to the self-scored method,the low dose of morphine was used at the beginning and then changed into Durogesic dose. It was plastered above the chest or abdomen wall and replaced with a new one or adjusting dose every 72 hours.Results Overall response rate of easing cancer pain was 70.9% (44/62).The efficacy for chronic continuous pain was better than that for immediate pain.The side effects were slight.The performance status had been improved with better appetite,sleep and spirit.Conclusion Fentany transdermal system is effective in relieving cancer pain ,especially useful for carcinoma with chronic pain and middle-severe pain.
