人工髋关节置换后的髋部疼痛

摘    要:
背景:人工全髋关节置换是治疗各类髋关节疾病的有效措施,但施行置换难度很大,且置换后疼痛等并发症越来越多,其诊断与处理争议颇多。目的:探讨人工全髋关节置换后髋部疼痛的原因及预防治疗措施。方法:收治人工全髋关节置换后疼痛患者21例(23髋),均为关节内疼痛,10例采用保守治疗,其余11例手术治疗,应用目测类比疼痛评分、Harris评分对随访结果进行置换后疼痛原因分析。结果与结论:21例均获随访,随访时间1~9年。关节内疼痛感染组3例中保守治疗2例、二期翻修1例,随访感染均治愈,置换后应用目测类比疼痛评分降低、Harri评分与置换前比较显著提高,差异有显著性意义(P < 0.05)。非感染组保守治疗8例,手术治疗10例,与置换前目测类比评分及Harri评分均好转比较,差异有显著性意义(P < 0.05)。

Abstract:
BACKGROUND:Total hip arthroplasty is an effective measure for the treatment of various hip diseases. Due to the difficult implantation, pain and other complications after replacement, there is controversy in the diagnosis and treatment.OBJECTIVE:To analyze the causes of hip pain after total hip arthroplasty and investigate relative prevention and treatment measures.METHODS:The data of 21 patients (23 hips) with hip pain after total hip arthroplasty were collected, among them, 10 patients received conservative treatment, while the other 11 patients were given surgical treatment. The visual analogue scale score and Harris hip score were used to analyze the cause of hip pain after total hip arthroplasty.RESULTS AND CONCLUSION:All the 21 patients were followed-up for 1-9 years. Among the 3 patients with intra-articular pain in the infected group, 2 patients were given conservative treatment and 1 was given second stage revision, infection was cured by follow-up, and postoperative visual analogue scale score was decreased while the Harris score was significantly increased when compared with those before replacement, and the difference was statistically significant (P < 0.05). In the non-infected group, 8 patients were treated with conservative treatment and 10 patients received surgical treatment, the visual analogue scale scone and Harris score were improved when compared with those before replacement, and the difference was significant (P < 0.05).
