腹压训练加间歇导尿法对脊髓源性非反射型膀胱的综合效果评价

目的探讨脊髓源性非反射型膀胱的最佳护理方案。方法将60例脊髓源性非反射型膀胱患者随机分A、B、C3组,每组各20例;A组采用腹压训练加间歇导尿法,B组采用腹压训练加留置导尿管法,C组采用间歇导尿法。于护理3周后评价3组患者疗效、尿路感染发生率及经济学指标。结果干预3周后,3组患者的疗效差异均有统计学意义(P<0.05),其中以A组最佳;B组患者尿路感染发生率高于A组和C组,差异有统计学意义(P<0.05和P<0.01);出院时医疗成本从低到高依次为A、B、C组。结论腹压训练加间歇导尿术可以让患者尽早形成平衡膀胱,减少泌尿系感染,降低医疗费用,是脊髓源性非反射型膀胱的最佳护理方案。

Comprehensive Effect Assessment of Abdominal Pressure Training and Intermittent Catheterization on Spinal Reflectionless-type Bladder

Objective To discuss the optimal nursing measure of spinal reflectionless-type bladder.Methods Sixty patients with spinal reflectionless-type bladder were randomly divided into three groups：group A（n=20）,group B（n=20）,group C（n=20）.Abdominal pressure training and intermittent catheterization were carried out for group A,abdominal pressure training and indwelling urethral catheterization for group B,and intermittent catheterization for group C.The effective rates,urinary tract infection rates and economic indicators of these 3 groups were evaluated 3 weeks after the nursing intervention.Results After 3 weeks of nursing intervention,significant differences were observed of the three groups（PB〉A.Conclusion Abdominal pressure training and intermittent catheterization can create balance bladder as early as possible,reduce the rate of urinary tract infection and pathology costs,which is the optimal nursing measure for spinal reflectionless-type bladder.
