双源CT和超声心动图对小儿紫绀型先天性心脏病的诊断比较

摘    要:目的比较双源CT和超声心动图显示小儿复杂先天性心脏病畸形的准确性和影响因素。方法回顾性分析作者医院行双源CT和超声心动图检查,并经手术确诊的125例3岁以下紫绀型复杂先天性心脏病患者资料,以手术结果为标准,比较两种检查方法显示左右肺动脉、主肺动脉、心内结构性畸形的能力,评价心率对双源CT诊断准确性的影响。结果双源CT显示发育不良肺动脉的能力优于超声心动图检查。两种检查方法诊断心内结构性畸形的准确性无明显差异,双源CT在心率120/min以下时测量小儿肺动脉的准确性更高。结论双源CT诊断小儿复杂先天性心脏的心外血管和心内结构性畸形的准确性更高。

Abstract:Objective To compare the accuracy and influential factors of dual-source CT(DSCT) and transthoracic echocardiography(TTE) in the diagnosis of cardiac abnormalities with complex congenital heart disease(CCHD) in children.Methods In 125 children under 3 years old with CCHD diagnosed from August 2009 to October 2011,TTE and DSCT were performed to determine the pulmonary diameters and intracardiac structural malformation.Their abilities in detecting the left and right pulmorary antery,aortopulmonary and intracandiac structural malformation were compared.The effects of heart rate on the accuracy of DSCT were evaluated.Results The accuracy of DSCT was superior to TTE in showing pulmonary dysplasia.There was no significant difference in diagnostic accuracy for intra cardiac structural abnormalities between the two methods.DSCT had higher accuracy when the heart rate was less than 120/min.Conclusion DSCT has better accuracy in diagnosing pulmonary dysplasia and intracardiac structural malformations of CCHD children.
