急性有机磷农药中毒患者尿微量清蛋白和视黄醇结合蛋白的变化及意义

目的 探讨急性有机磷农药中毒（AOPP）患者尿微量清蛋白（MA）和视黄醇结合蛋白（RBP）检测的意义。方法 用速率散射比浊法检测71例AOPP患者尿MA含量，用ELISA法检测尿RBP含量，并与健康对照组比较。结果 71例AOPP患者中，20例中毒后尿MA和RBP逐渐增高，且随着病情的进展尿蛋白呈阳性；51例尿蛋白阴性患者尿MA含量增高者16例，尿RBP含量增高者18例，在中毒后第1、3、5天尿MA含量中位数分别为2．12、2．31、1．70mg／mmool Cr，均高于健康对照（1．25mg／mmool Cr），差异有统计学意义（P〈0．05），且中毒愈重，尿MA增高愈明显。中毒后第1、3、5、7天尿RBP含量中位数分别为10．1、42．3、22．7、9．1mg／mol Cr，均高于健康对照（8．1mg／mol Cr），差异有统计学意义（P〈0．05），且中毒愈重，尿RBP增高愈明显。结论 尿MA和RBP对急性有机磷农药中毒患者肾损害具有早期诊断价值。

Alterations of microalbuminuria and retinol binding protein and their significance in patients with acute organophosphorus pesticide poisoning

Objective To explore significance of microalbuminuria(MA) and retinol binding protein(RBP) in acute organophosphorus pesticide poisoning(AOPP).Methods MA and urinary RBP were detected by rate scatter nephelometry and enzyme-linked immunosorbent assay(ELISA) respectively in 71 patients with AOPP and 45 healthy controls.Results Out of 71 patients with AOPP,20 patients'MA and RBP were higher and urinary albumen was positive with the development of poisoning.16 patients' MA was higher and 18 patients'urinary RBP was higher in 51 patients with urinary albumen negative.The medium of MA median was 2.12,2.31,1.70 mg/mmol Cr on day 1,3,5 respectively after poisoning,higher than that of healthy controls(1.25 mg/mmol Cr)(P<0.05),the more serious poisoning was,the higher was MA.The medium of urinary RBP level on day 1,3,5,7 after poisoning was 10.1,42.3,22.7,9.1 mg/mol Cr respectively,significantly higher than that of healthy controls(8.1 mg/mol Cr,P<0.05).The more serious poisoning was,the higher was urinary RBP.Conclusion MA and urinary RBP might be of early diagnositic value in kidney injury of AOPP.
