三维重建在肝切除术前评估的临床应用研究

目的探讨三维重建在肝切除术前评估的临床应用价值。方法按一定纳入标准选择行肝脏部分切除术的20例患者作为研究对象,术前进行三维重建评估,并将评估结果及手术预案与术中实际情况进行对比分析。结果 20例患者均成功进行手术,完整的切除肿瘤。本组病例无手术死亡和住院期间死亡,平均手术时间(260±68)min,平均术中出血(350±180)mL,无重大手术并发症发生。结论术前评估结果及与手术预案与临床实际情况比较接近,提示三维重建在肝切除术中具有重要的临床应用价值,可以为精确、安全施行复杂的肝脏大部分切除术制定合理的手术方案和采取正确的术中策略提供重要的术前参考,最大限度减少术后肝功能障碍的发生。

Study on the clinical application of preoperative evaluation of fiver using three - dimensional reconstruction

Objective To investigate the clinical effect of preoperative evaluation of liver using three - dimensional reconstruction. Meth- ods Twenty consecutive patients for liver resection were collected into this study. Pre - operative evaluation of liver using three - dimensional reconstructionwere performed, then, Compared and contrasteted the preoperative evaluation of liver and operation planning with peri - operative and post - operative actual conditions. Results The 20 patients have all done the operation successfully and the tumor was resected completely. In this study there was no one died in operation and hospital stay. The mean operative time was 260 ± 68rain , the mean blood loss was 350 ± 180ml and no severe surgical complication occurred. Conclusion The ideal simulation effect can be achieved compared the virtual image with actual operation. This implied that the preoperative evaluation of liver using three - dimensional reconstruction had significant clinical effect. It could provide the important preoperative reference for avaluable planning and intraoperative strategy of complex hepatectomy.
