电视胸腔镜微创诊治孤立性肺结节

摘    要:目的 探讨用电视胸腔镜手术对孤立性肺结节的微创伤诊治方法。方法 1994年10月至2003年3月电视胸腔镜下切除孤立性肺结节47例。结果 除1例炎性假瘤因胸腔内较严重粘连、中转开胸手术外,其余均在电视胸腔镜下顺利完成肺楔形切除术。手术时间、术后留置胸腔引流管时间和住院时间分别平均为26.8min。、1.2天和3.9天。全组无手术死亡,无与手术相关的术中术后严重并发症。结论 电视胸腔镜手术是诊治孤立性肺结节的安全有效方法,具有明显的微创优势。

Abstract:Objective To evaluate the clinical effect of minimally invasive technique with videoassisted thoracoscopy in diagnosis and treatment of solitary pulmonary nodule (SPN, diameter <3 cm).Methods During October 1994 to March 2003, 47 patients with SPN were performed pulmonary wedge resection through videoassisted thoracoscopic surgery. Results All of the patients were performed videoassisted thoracoscopic pulmonary wedge resection successefully except 1 patient with inflammatory pseudotumor was failed in thoracoscopic procedure due to pleural adhesion and then treated with open surgery. The mean operating time, duration of postoperative chest tube drainage and in hospital time were 26.8 minutes, 1.2 days and 3.9 days respectively. In this treatment the death and serious perioperative complications were not found. Conclusion In the diagnosis and treatment of SPN, the videoassisted thoracospic surgery is a safe, effective, timesaving and minimally invasive approach.
