人乳头瘤病毒基因分型检测在宫颈病变早期诊断中的应用探讨

摘    要:
目的探讨女性人乳头瘤病毒(HPV)基因分型检测与宫颈上皮内瘤变(CIN)的关系。方法应用DNA杂交技术对3591例妇科门诊就诊者进行宫颈HPV分型检测,并对其中270例高危型HPV感染患者的宫颈上皮组织活检确定病变程度。结果 3591例样本中,HPV感染837例(23.31%),HPV感染人次1233人次,检测高危型HPV感染(16、18、31、33、35、39、45、51、52、53、56、58、59、66、68、73、83)906人次(73.48%),高危型感染率前5位是HPV16、58、52、18、33;检出低危型HPV感染(6、11、42、43、81)327人次(26.52%)。270例高危型HPV感染患者中检出低度鳞状上皮内瘤变(LSIL)13例,高度鳞状上皮内瘤变(HSIL)41例。结论 HPV基因分型检测对宫颈病变的早期诊断有重要意义,可作为宫颈癌筛查手段。

Abstract:
【Objective】To explore the correlation between the genotyping of human papillomavirus and cervical intraepithelial neoplasia(CIN).【Methods】Totally 3591 were detected for HPV gentotyping by using DNA hybridization technique,and cervical biopsies were used for pathological diagnosis in 270 cases of high-risk HPV infection.【Resluts】 Among 3591 cases,837 cases were HPV positive(23.31%).Among 1233 cases of HPV infection 906(73.48%)were infected with high-risk HPV types(HPV-16,18,31,33,35,39,45,51,52,53,56,58,59,66,68,73,83),The HPV high-risk infection rate in top 5 were HPV 16,58,52,18 and 33 in turn.327(26.52%) were infected with low-risk HPV types(HPV —6,11,42,43 and 81),Among 270 cases of high-risk HPV infection,13 cases were low grade squamous intraepithelial lesion(LSIL) and 41 cases were high grade squamous intraepithelial lesion(HSIL).【Conclusion】The detection of HPV genotypes is of great significance for early diagnosis ofcervical lesion andtherefore for the prevention and treatment of cervical cancer.
