早期康复训练对重型颅脑损伤患者预后的影响

目的：探讨早期康复护理训练对重型颅脑损伤患者生存质量的影响。方法：对收治的100例重型颅脑损伤病人随机分为两组：早期康复训练组52例；常规护理组48例。对其进行治疗前和治疗后格拉斯哥昏迷指数（GCS）评分，治疗后6月进行格拉斯哥结果指数（GOS）分级，并对二组进行比较。结果：治疗前GCS评分二组比较差异无统计学意义，治疗后实验组GCS评分平均提高了（3．97±1．76）分，对照组GCS评分平均提高了（2．91±1．84）分，二者比较差异存在统计学意义（P〈0．01）；实验组良好率（GOSⅣ级和Ⅴ级）较对照组高32％，病死率、植物生存率较对照组下降30％，两组比较，实验组生存质量明显优于对照组（P〈0．05和P〈0．01）。早期康复训练组疗效优于常规护理组（P〈0．05）。结论：早期系统化康复护理可促进重型颅脑损伤患者神经功能恢复，改善预后。

THE CLINICAL EFFECT OF EARLY REHABILITATION CARE IN TREATMENT OF SEVERE BRAIN INJURY

Objective：To explore the clinical effect of early rehabilitation care in treatment of severe brain injury. Methods：100 patients suffering from severe brain injury were randomly divided into two groups：early rehabilitation care group （52 patients） and control group （48 patients）.Both groups were given the conventional treatment.The change of Glasgow Coma Scale （GCS） before and after treatment and Glasgow Outcome Scale （GOS） after 6 months were compared. Results：GCS in the two groups before treatment were not significantly different （P〉0.05）.After treatment the GCS in the early rehabilitation care group was improved by average of 3.97±1.76 in contrast to 2.91±1.84 in the control group. There was statistic significance between two groups （P〈0.01）. The rate of good outcome （GOSIV and V） were 32% higher in study group than in control group. The mortality and vegetable survival rate were 30% lower in the early rehabilitation care group than in the control group.The survival quality of study group is better than the control group （P〈0.05 ,P〈0.01 ）. The therapeutic effect of early rehabilitation care group is better than the usual care group （P〈0.05）. Conclusion：The early systematical rehabilitation care has a positive significance in the recovery of neurological function of patients with severe brain injury, and improving the quality of the patient＇s life.
