奥曲肽治疗食管胃静脉曲张出血的疗效观察

目的观察两种剂量奥曲肽治疗食管胃静脉曲张出血的疗效和安全性。方法回顾性分析2002年1月-2010年2月消化内科100例应用奥曲肽治疗的食管胃静脉曲张出血病例,其中50例奥曲肽剂量为25μg/h,50例奥曲肽剂量为50μg/h,比较两组患者的止血时间、止血率、再出血率及不良反应。结果止血时间：奥曲肽50μg/h剂量组为（12.0±10.5）h,明显短于奥曲肽25μg/h剂量组为（19.5±16.5）h,P〈0.05;6h、12h、24h、48h和72h止血率：奥曲肽50μg/h剂量组为56%、78%、88%、90%和92%,明显高于奥曲肽25μg/h剂量组的38%、54%、70%、74%和78%（P〈0.05）;治疗总有效率：奥曲肽50μg/h剂量组为92%,明显高于奥曲肽25μg/h剂量组的78%;再出血率：奥曲肽50μg/h剂量组为10%,明显低于奥曲肽25μg/h剂量组的22%（P〈0.05）,差异有统计学意义,两组均未出现明显不良反应。结论两种剂量的奥曲肽对食管胃静脉曲张破裂出血均有效,奥曲肽50μg/h剂量组疗效优于奥曲肽25μg/h剂量组,止血时间短,再出血率低,无明显不良反应。

Effect and Safety of Two Dosages of Octreotide for the Treatment of Esophageal Gastric Variceal Bleeding

Objective To observe the effect and safety of two dosages of octreotide for the treatment of esophageal gastric variceal bleeding（EGVB）.Methods The data of 100 patients who received octreotide for the treatment of EGVB were retrospectively analyzed.100 patients was divided by two groups：25 μg/h,50 μg/h.Time of relieving bleeding、rate of relieving bleeding and rate of rebleeding were compared between the two groups.Results Time of relieving bleeding：the 50 μg/h group＇s was（12.0±10.5）h,and much less than the 25 μg/h group＇s：（19.3±16.5）h.Rate of relieving bleeding：the 50 μg/h group＇s at 6 h,12 h,24 h,48 h,72 h were 56%,78%,88%,90%,92%,and much higher than the 25 μg/h group＇s：38%,54%,70%,74%,78%.Rate of rebleeding：the 50 μg/h group＇s was 10%,much less than the 25 μg/h group＇s：22%.Conclusion Two dosages of octreotide on EGVB was effective,and the 50 μg/h group was more effective than the 25 μg/h group.
