流式细胞术检测4272例人类白细胞抗原-B27表达的临床意义

目的]通过流式细胞术检测人类白细胞抗原B27(HLA-B27)在各类疾病患者中的表达,评价HLA-B27的临床应用价值。方法]用流式细胞仪分析2008年1月1日至2011年6月27日杭州各大医院4272例入院就诊患者外周血T淋巴细胞HLA-B27抗原表达。结果]HLA-B27表达阳性有1209例,另有5例阳性结果需进一步确认,阳性率为28.30%,其中男性阳性率为34.58%,,女性为20.36%;阳性患者中主要以21岁至40岁为主,按年龄组分成≤20、21～30、31～40、41～50、≥50岁,男性阳性率分别为11.76%、36.12%、26.42%、13.94%和11.76%,女性分别为7.03%、25.78%、32.03%、15.36%、20.31%;强直性脊柱炎(AS)、类风湿性关节炎(RA)、风湿病、腰椎间盘突出症等疾病HLA-B27表达阳性率分别为89.25%、54.55%、47.82%、20.75%。结论]HLA-B27阳性表达率男性高于女性,主要集中于21～40岁,且与AS具有高度相关性。

Significance of the Expression of Human Leukocyte Antigen B27 in 4272 Patients with Flow Cytometry

Objective ] The expression of human leukocyte antigen B27 （HLA-B27） in patients with various diseases was detected by Flow Cytometry to evaluate the clinical value of HLA-B27. Methods] The expression of HLA -B27 antigen of peripheral blood T lymphocytes in 4272 hospital admission patients was analyzed by Flow Cytometry. Results] 1209 patients were positive in the 4272 hospital admission patients, and another five patients with positive results need further confirmation. The HLA-B27 positive rate was 28.30%（1209/4272）, of which men was 34.58%（825/2386） and women was 20.36%（384/1886）. The age of the HLA -B27 positive patients was mainly between 21 and 40 years old. Composed by age ≤20,21- 30,31-40,41-50, ≥50 years old, the positive rates of male and female were 11.76%, 36.12%, 26.42%, 13.94%, 11.76%（97,298, 218,115,97/825）, 7.03%, 25.78%, 32.03%, 15.36%, 20.31% （27,99,123,59,78 / 384）]. The positive rates of HLA-B27 expressing in ankylosing spondylitis （AS）, rheumatoid arthritis （RA）, rheumatoid arthritis and lumbar disc herniation patients were 89.25% （83/93）, 54.55% （36/66）, 47.82% （11/23） and 20.75% （22/106）, respectively. Conclusion] The positive patients of HLA-B27 were higher in men than women, and mainly focused on 21 to 40 years old, and the HLA-B27 positive rate was highly correlated with AS.
