Therapeutic effect and mechanisms of alpha lipoic acid in the treatment of diabetic peripheral neuropathy

Diabetic neuropathy is a multiple-factorial disorder. Oxidative stress is a critical factor in the development of diabetic perpheral neupropahy （DPN）. As a powerful antioxidant, alpha lipoic acid （ALA） has been used alone or combined with other drugs to treat DPN with good safety and tolerability. The protective mechanisms of ALA in treatment of DPN are involved in increase of endoneurial blood flow and the Na＋ -K＋ ATPase activity in nerve, improvement of nerve conduction velocity, inhibition of lipid peroxidation, resistance of inflammation and protection of endothelial function, etc.
