内镜Lugol液染色在早期食管癌及非典型增生诊断的应用

目的:为探讨Lugol液对早期食管癌、非典型增生的诊断价值。方法:对135例食管有表浅病变者进行Lugol液染色,同时进行活组织病理检查。结果:135例中不着色91例,其中食管癌18例(鳞癌16例,腺癌2例),食管黏膜慢性炎12例,食管息肉3例,Barrett食管14例,中度不典型增生28例,重度不典型增生6例,食管溃疡10例;浅染色23例,其中慢性食管炎11例,Barrett食管2例,中度不典型增生3例,食管溃疡6例,食管息肉1例;过度染色15例,鳞状上皮增生12例,慢性食管炎3例;正常染色6例,乳头状瘤2例,慢性食管炎2例,正常鳞状上皮2例。结论:Lugol液染色可以准确定位活检,对诊断早期食管癌及非典型增生有重要意义。

Application of Endoscopic Lugol's Solution in the Diagnosis of Early Esophageal Carcincma and Atypical Hyperplasia

Objective:To investigate the diagnostic valve of lugol's staining in early esophageal carcinoma and atypical hyperplasia.Methods:2% Lugol's solution was used to stain esophageal mucosa in 135 cases with esophageal superficial lesions and multiple biopsies were taken for pathological examinations.Results:Among 135 cases using lugol's staining,91 cases had no staining,which included 18 early esophageal cancers,16 squamous cancers,2 adenoid cancers,12 chronic esophagitis,3 esophageal polypus,14 Barrett's esophagus,28 moderate atypical hyperplasia,6 severe atypical hyperplasia,10 esophageal ulcer.23 cases were mildly stainned,which included 11 chronic esophagitis,2 Barrett's esophagus,3 moderate atypical hyperplasia,6 esophageal ulcer and 1 esophageal polypus.Deep staining showed in 15 cases including 12 squamous epithelium proliferation and 3 chronic esophagitis.Normal staining was found in 2 papilloma,2 chronic esophagitis and 2 normal squamous.Conclusion:Lugol's staining is accurate in positionng biopsy.It is valuable in diagnosing early esophague cancer and atypical hyperplasia.
