老年人风湿性心脏病42例临床分析

本文对以二尖瓣病变为基础的42例老年风心病与同期42例青中年风心病作临床对比分析。老年风心病特点为:临床表现轻、风湿活动隐匿。有风湿史、心尖部Ⅱ级以上舒张期杂音、风湿再活动、肺动脉第二音亢进,分别为38.3％、19％、7.1％和2.4％;患者均有一种以上的合并症和并发症;房颤、其它心律失常和脑栓塞发生率高为92.8％与11.9％;强心甙中毒和精神症状较多见为14.3％与7.1％;本文通过3例手术后疗效观察指出:促使老年风心死亡率下降的一个因素是尽早手术。

RHEUMATIC HEART DISEASE IN THE AGED (A CLINICAL ANALYSIS OF 42 CASES)

Forty-two cases of rheumatic heart diseases in the aged and forty-two cases in the young and middle aged were analysed for comparing with each other. The clinical manifestations are relatively milder and rheumatic activity is latent. The morbidity of rheumatic history, rheumatic reactivity, lound pulmonary S_2, diastolic murmur over grade Ⅱ in apex, auricular fibrillation and other arr- hythmias, cerebral embolism, digitalis intoxication, and symptoms of psychosis are 38.3%, 7.1%, 19%, 2.4%, 92.8%, 11.9%, 14.3%, and7.1% respectively. Three cases were operated on and it is believed that early operation can reduce the mortality of rheumati heart diseases in the aged.
