清肝利胆汤加减辅助西药治疗急性胆囊炎肝胆湿热证临床研究

目的：观察清肝利胆汤加减辅助西药治疗急性胆囊炎肝胆湿热证的临床疗效。方法：采用随机数字表法将96 例急性胆囊炎肝胆热证患者分为试验组、对照组各48 例。对照组接受常规西药治疗,试验组在对照组基础上加用清肝利胆汤加减治疗,2 组均连续治疗7 d。比较2 组临床疗效、临床症状消失时间及治疗前后中医证候积分、炎症指标[白细胞计数（WBC）、C-反应蛋白（CRP）、肿瘤坏死因子-α（TNF-α） ]水平。结果：试验组总有效率91.67%,高于对照组75.00%（P＜0.05）;发热、右上腹痛、腹部压痛、胃肠道症状消失时间均短于对照组（P＜0.05）。2 组治疗后中医证候积分、WBC、CRP、TNF-α 水平均较治疗前降低（P＜0.05）,且试验组治疗后中医证候积分、炎症指标水平均低于对照组（P＜0.05）。结论：清肝利胆汤加减辅助西药治疗急性胆囊炎肝胆湿热证效果显著,能够加快患者恢复,减轻炎症反应。

Clinical Study on Modified Qinggan Lidan Decoction Assisting Western Medicine forAcute Cholecystitis with Liver-Gallbladder Damp-Heat Syndrome

Abstract： Objective： To observe the clinical effect of modified Qinggan Lidan Decoction assistingwestern medicine on acute cholecystitis with liver-gallbladder damp-heat syndrome. Methods：A total of96 cases of patients with acute cholecystitis with liver-gallbladder damp-heat syndrome were divided intothe trial group and the control group according to the random number table method,with 48 cases in eachgroup. The control group was treated with routine western medicine,and the trial group was additionallytreated with modified Qinggan Lidan Decoction based on the treatment of the control group. Both groupswere treated for 7 days. Traditional Chinese medicine (TCM) syndrome scores and the levels ofinflammation indexes [white blood cell count (WBC),C-reactive protein (CRP),and tumor necrosis factor-α (TNF-α)] before and after treatment,as well as the clinical effects and the time for elminating the clinicalsymptoms were compared between the two groups. Results：The total effective rate was 91.67% in thetrial group, higher than that of 75.00% in the control group (P＜0.05). The disappearance time of fever,right upper abdominal pain,abdominal tenderness and gastrointestinal symptoms was shorter than that inthe control group (P＜0.05). After treatment, TCM syndrome scores and the levels of WBC, CRP andTNF- α in the two groups were decreased when compared with those before treatment (P＜0.05), andTCM syndrome scores and inflammation indexes in the trial group were lower than those in the control group (P＜0.05). Conclusion： Modified Qinggan Lidan Decoction assisting western medicine for acutecholecystitis with liver-gallbladder damp-heat syndrome has a significant effect,which can accelerate therecovery and reduce inflammatory responses.
