40例进展期胃癌术后胃镜观察

 目的　探讨胃癌术后残胃的发病情况及病因。方法　40例胃癌术后内镜观察分析。结果　残胃复发癌20%，反流性食管炎13%，吻合口炎17%，残胃炎20%，吻合口线头5%，吻合口狭窄7%，吻合口溃疡10%，残遗癌8%。残胃复发癌发生与胆汁反流及局部术后血供障碍有关外，还与消化道重新建方式、癌的浸润程度、胃周围淋巴结有无转移及清除的范围有关。结论　我们认为对胃癌术后患者定期进行内镜检查，就有可能早期发现复发癌。

Endoscopy of 40 Cases after Radical Operation for Advance Carcinoma of Stomach

To probe into the main cause and charcteristics of disease with stump stomach after radical gasteretomy for carcinoma of stomach.Methods An Analgsis of Endoscopy of 40 cases after radicaloperation for carcinoma of stomach.Results Recurrence of the cancer is 20%,inflammation of stump stomach 20%,Esophagitis Reflux 13%,Anastomotic suture 17%,Stump stomach 20%.Suture Line in Anastomotiostoma 20%.Sutue Line in Anastomo stoma 5%,stomal narrow 7%,stomal ulcer 10%,Residual Cancers 8%.The relationship between Recurrene of the cancer the bilie reflux,bloodseream deterrent of cocalized operation and types of gastric surgical procedure,the depth of cancer invasion,metastasis to the regional lymph nodes.and Residual cancers were annlyzed.Conclusion In case that hed undergone surgery for cancer.diagnosis of recurrence is extremely important there-fore,It is necessary to be very observant by endoscopy.
