无血切肝技术的改进--选择性肝静脉阻断在肝切除术中的应用(附11例报告)

目的 改进无血切肝技术，提高肝脏巨大肿瘤的切除率。方法 回顾性总结11例肝脏巨大肿瘤切除术，讨论选择性肝静脉阻断方式在肝切除中的应用。结果 巨大肝脏肿瘤在肝切除术中应用选择性肝静脉阻断术，术中出血少，术式安全可靠，不需要特殊的器械和仪器，提高了肝脏手术的切除率，使常规手术无法切除的肝脏巨大肿瘤得以切除，为肝脏手术提供了一个科学合理的安全术式。结论 应用选择性肝静脉阻断可以提高肝脏巨大肿瘤的切除率。

Application of selective exclusion of hepatic veins,an improvement of bloodless hepatectomy,in hepatectomy

Objective To improve the technique of bloodless hepatectomy to elevate the resectable rate of massive hepatic tumors. Methods The clinical data of 11 cases of massive hepatic tumors treated in our hospital were retrospectively analyzed to discuss the application of selective exclusion of the hepatic veins in hepatectomy. Results Only 1 patient received transfusion of 400 ml blood during the operation. Application of selective exclusion of the hepatic veins in hepatectomy provided a safe operative procedure for hepatic surgery. Conclusions Use of selective exclusion of the hepatic veins can increase the rate of resection for massive hepatic tumors.
