胃镜直视下食管扩张联合药物应用治疗食管癌术后吻合口狭窄36例临床分析

目的探讨胃镜直视下食管扩张联合药物应用治疗食管癌术后吻合口狭窄的方法及疗效。方法选择2007年7月到2009年9月间食管癌术后发生吻合口狭窄患者36例,进行胃镜直视下用食管扩张器食管扩张联合2%高渗盐水加地塞米松治疗。结果 36例患者中1次治疗后显效29例,占80.5%,有效5例（占13.9%）,总有效率达94.4%;术中、术后未出现消化道穿孔、大出血、吻合口瘘等并发症。结论胃镜直视下食管扩张器食管扩张联合药物应用治疗食管癌术后吻合口狭窄方法简单,疗效满意,值得推广应用。

Clinical analysis on esophageal dilatation guided by endoscopy combined with the application of drugs for the treatment of postoperative anastomotic stenosis in 36 cases with esophageal cancer

Objective To discuss the method and effect of esophageal dilatation guided by endoscopy combined with the application of drugs for the treatment of postoperative anastomotic stenosis in cases with esophageal cancer. Methods 36 cases of esophageal cancer patients with postoperative anastomotic stenosis con-firmed through barium through the esophagus and gastroscopy were treated using esophageal dilatation guided by endoscopy combined with the application of drugs. Results Total efficiency was 94. 4% （ 34 /36） ,there were 29 cases with marked effect ,1 case with effect after once of treatment ,without complications such as gastroin-testinal hemorrhage and perforation and anastomotic fistula. Conclusion The method of esophageal dilatation guided by endoscopy combined with the application of drugs for the treatment of postoperative anastomotic stenosis in cases with esophageal cancer is simple,cheap,safe and reliable. Thus this method is well worthy of spreading to application.
