2型糖尿病患者低血糖反应的原因分析

目的分析2型糖尿病患者出现低血糖的相关危险因素。方法回顾2010年9月至2011年12月重庆医科大学附属第二医院内分泌科2型糖尿痛住院患者病例，采用两样本的t检验方法，对120例研究对象进行了分析，收集性别、年龄、糖尿病病程、体质量指数、丙氨酸氨基转移酶、血肌酐、内生肌酐清除率、糖化血红蛋白、糖尿病肾病分型、治疗方式及胰岛素治疗年限、合并其他非降糖类药物等数据，对收集的变量进行方差分析、卡方检验。结果与2型糖尿病低血糖发生有统计学意义的危险因素是年龄、糖尿病病程、血肌酐、内生肌酐清除率、糖化血红蛋白、糖尿病肾病分型、治疗方式及胰岛素治疗年限，以及合并使用拜阿司匹林。结论病程长、高龄、低体质量、低糖化血红蛋白、肾功能异常、磺脲类药物及胰岛素治疗超过5年、合并使用拜阿司匹林等其他非降糖类药物、低文化及性别是住院2型糖尿病患者发生低血糖的独立危险因素。

Study on the risk factors of hypoglycemia in type 2 diabetes mellitus

Objective The explore the risk factors of hypoglycemia in type 2 diabetic patients. Methods Two independent sample t-test was conducted, on 120 pairs of subjects which were observed from September of 2010 to June of 2011 in the second affiliated hospital of Chongqing medical university. Information was including gender,age, duration of diabetes, body mass index （BMI）, glutamic-pyruvic transaminase, creatinine （Cr）, endogenous creatinine clearance rate（Ccr）,glyceated hemoglobin AIC（HbAlc）, treatment, etc. Chi-square test and variance analysis were used to analyze the data. Results The risk factors,which was significantly associated with type 2 diabetes were age, duration of diabetes, Cr, Ccr, HbAlc, diabetic nephropathies, treatment, and use of bayaspirin （P 〈 0. 05 ）. Conclusion Duration of diabetes,age,BMI,HbAlc,abnormal renal function,use of bayaspirin（more than 5 years）, combination use with bayaspirin and other non-antigyperglycemic drug,lack of education and sex are the independent risk factors of hypoglycemia in type 2 diabetes.
