社区门诊高血压诊疗和路径管理研究

目的:探索高血压社区门诊诊疗和慢病管理的新途径,以提高医疗质量和慢病管理水平。方法:以中国高血压防治指南(2005-2010)为基础,应用信息化手段以路径形式制定高血压社区门诊的诊疗规范。同时在路径中纳入高血压慢病规范化管理的内容,使高血压社区门诊和慢病管理活动有机结合。结果:路径实施提高了医疗质量和安全,优化了服务流程,控制了医疗费用不合理增长,提高了高血压慢病管理质量和效率。结论:社区门诊实施高血压诊疗和路径管理,是预防和控制高血压的有效方法,对有效开展高血压慢性病防控有十分重要的意义。

Study on hypertension diagnosis and treatment and clinical pathway management in community clinics

Objective： To explore a new pathway for hypertension diagnosis and treatment in out-patient department of community and chronic disease management for better medical quality and the high level of chronic disease management. Methods： On the basis of Chinese Hypertension Prevention Guide （2005-2010）, the criteria of hypertension diagnosis and treatment in the out-patient of community were established by the means of information technology and in the forms of clinical pathway. The standardized management of hypertension and chronic diseases would be included in the pathway, which would effectively combine the activities of hypertension clinical treatment and chronic diseases manage- ment. Results： The implementation of clinical pathway strengthens the medical quality and safety, optimizes the service process, controls the unreasonable growth of medical expenditure and increases the management quality as well as the effi- ciency of hypertension and chronic diseases. Conclusion： The implementation of hypertension diagnosis and treatment and pathway management in the community out-patient clinics is the effective approach for the prevention and control of hypertension, which plays a significant role in effective prevention and control of hypertension and chronic diseases.
