我院静脉用药集中调配中心不合理医嘱分析

目的：统计分析我院外科静脉用药集中调配中心不合理医嘱，为临床合理用药提供参考。方法：对我院外科药房静脉用药集中调配中心2012年7月的全部静脉用药医嘱进行统计分析。结果：共审查出2096条不合理医嘱，占医嘱总数的0．83％，不合理医嘱类型主要涉及：溶媒选择不当、给药频次不适宜、载体量不适宜、超出加液量、给药剂量不适宜、配伍禁忌等。结论：我院外科药房静脉用药集中调配中心用药医嘱存在一定的不合理现象，药师应积极发挥对不合理医嘱的干预作用。以促进临床更为合理的用药。

Analysis of irrational medical advices in the pharmacy intravenous admixture services of our hospital

Objective： To analyze statistically the irrational medical advices in the pharmacy intravenous admixture services （PIVAS） of surgical pharmacy in our hospital, and parovide references for clinical rational drug use. Methods： The medical advices of PIVAS in the surgical pharmacy in our hospital were summarized and analyzed statistically in July 2012. Results： A total of 2096 irrational medical advices were reviewed, accounting for 0.83% of the total medical advices. The types of the irrational medical advices contained incorrect selection of solvent, improper frequency and carrier amount, excessive liquid adding amount, improper dosage, incompatibility and so on. Conclusion： Based on the problem of irrational medical advices in PIVAS, pharmacists should play an important role in intervening the irrational medical advices for promoting the rational drug use.
