液基细胞学联合阴道镜早期诊断宫颈癌的临床研究

目的：探讨液基细胞学联合阴道镜诊断早期宫颈病变的临床价值。方法：采用回顾性分析的方法，选择2005-2011年本院妇产科经液基细胞学筛查，后行阴道镜检查诊断并取活检作病理检查的患者共526例，统计其检查结果，以病理学诊断为金标准，比较单纯TCT或阴道镜检查与两者联合检查之间的差异。结果：TCT的灵敏度为85．8％，特异度为95．5％；阴道镜检查的灵敏度为91．6％，特异度为98．8％。液基细胞学联合阴道镜检查双阳性的阳性预测值为100％，双阴性的阴性预测值也为100％，灵敏度为99．5％，特异度为95．5％。结论：液基细胞学筛查宫颈癌能够提高标本质量，可作为宫颈癌的早期筛查手段，联合阴道镜检查能显著提高宫颈异常细胞的诊断率，有助于宫颈癌前病变及早期宫颈癌的早诊断早治疗，提高生存率。

Application of TCT Combined with Colposcopy in Diagnosis of Cervical Lesions

Objective： To explore and -evaluate the clinical value of thinprep cytology test （ TCT ）eombined with colposcopy in diagnosis of cervical lesions.Method： The diagnostic criteria for pathological results in our hospital colposcopy, biopsy and the cervical lesions TCT test results were analyzed.Result： The sensitivity of TCT was 85.8% and the specificity was 95.5%.The sensitivity of colposcopy was 91.6% and the specificity was 98.8%. In TCT with colposcopy, the positive predictive value of double positive results was 100% and the negative predictive value of double negative result was 100%, too.The sensitivity of TCT with colposcopy was 99.5%and the specificity was 95.5%.Conclusion： Application of TCT assisted colposcopy can help tomake early diagnosis and treatment for cervical precancerous lesion which improve the survival rate.
