人工肱骨头置换与切开复位钢板内固定治疗肱骨近端粉碎性骨折的对比研究

目的对比分析人工肱骨头置换与切开复位钢板内固定治疗肱骨近端粉碎性骨折的临床效果及其临床应用价值,以探讨治疗肱骨近端粉碎性骨折的较理想的治疗方法。方法分析2002年1月至2005年10月在我院骨科因肱骨近端粉碎性骨折手术的病例并归列为两大组:人工肱骨头置换组(简称假体置换组)与切开复位钢板内固定组(简称切开固定组)。术后采用Constant评分标准计算两组的优良率,通过优良率比较两组的近期疗效,同时对两组手术时间、术中出血量、术后肩关节部分活动度的恢复情况等应用SPSS13.0统计软件进行统计学分析比较,组间比较采用t检验,计数资料采用χ2检验,以评价两组近期疗效及其相关影响因素。结果进行人工肱骨头置换组48例,随访40例。疗效优16例、良18例、可3例、差3例,优良率85.0%。切开复位内固定组43例,随访36例。疗效优9例、良12例、可8例、差7例,优良率58.3%。同时,人工肱骨头置换组相对切开复位内固定组具有手术时间短和较少的术中出血量,较好的术后关节活动恢复情况,两组间差异有显著性。结论人工肱骨头置换术治疗肱骨近端粉碎性骨折疗效优于切开复位钢板内固定治疗方法,并且近期功能恢复快、并发症少,是治疗肱骨近...
