胰腺癌患者血清骨桥蛋白的检测及其临床意义

目的 探讨血清骨桥蛋白检测在胰腺癌诊断、转移评价和临床分期中的应用价值.方法 应用酶联免疫吸附法检测胰腺癌患者57例、胰腺良性疾病患者53例和正常人50例的血清骨桥蛋白（OPN）水平,比较各组之间OPN水平的差异.通过ROC曲线确定血清OPN临界值,计算OPN指标用于胰腺癌临床诊断和远处转移评价的敏感性和特异性.结果 胰腺癌患者血清OPN水平显著高于胰腺良性疾病患者和正常人群（P〈0.05）,而后两组之间OPN水平无明显差异（P〉0.05）.血清OPN水平变化与胰腺癌远处转移以及临床分期有关.血清OPN诊断胰腺癌的敏感性和特异性分别为83.0%和61.0%,预测肿瘤远处转移的敏感性和特异性分别为89.0%和57.0%.结论 血清OPN水平检测有助于胰腺癌的诊断、远处转移的预测和临床分期评估.

Measurement of serum osteopontin in patients with pancreatic cancer and its clinical significance

Objective To evaluate serum osteopontin levels in diagnosis, metastasis prediction and stage evaluation of pancreatic carcinoma. Methods Serum osteopontin was measured in 57 cases of pancreatic carcinoma, 53 cases of benign pancreatic disease, and 50 normal controls with enzyme-linked immunosorbent assay （ELISA）. The cut-off value of serum osteopontin was determined according to ROC curve, and the sensitivity and specificity for the diagnosis and metastasis prediction of pancreatic carcinoma were also calculated respectively. Results The level of serum osteopontin in pancreatic cancers was significantly higher than that in both benign pancreatic diseases and normal controls （P 〈 0. 05）, but there was no difference between the later two groups （P〉0.05）. The osteopontin levels were associated with the metastasis and clinical stages of pancreatic cancers. The sensitivity and specificity of serum osteopontin were 83% and 61% respectively for the pancreatic cancer diagnosis, and 89% and 57% for the metastasis prediction. Conclusion Measurement of serum osteopontin is of value for the diagnosis, metastasis prediction and stage evaluation of pancreatic cancers.
