颅脑手术后持续性植物状态患者促醒性的治疗

目的 探讨临床药物促醒性治疗的疗效预测方法，旨在建立脑电图反应性与颅脑手术后持续性植物状态（PVS）患者预后关系。方法 将40例PVS患者按其静脉滴注盐酸纳洛酮的脑电图反应情况分为甲乙两组，甲组为脑电图有明显反应者，乙组为脑电图无明显反应者，其后给予相同剂量的治疗，观察并分析疗效。结果 甲组12例患者中9例促醒成功，意识转清率为75.0％;乙组28例患者中3例促醒成功，意识转清率为10．7％；统计学分析认为甲组意识转清率明显高于乙组。结论 利用患者耐促醒性药物的脑电图反应情况可以大致评价PVS患者的预后，其弥补了按Synek脑电图分级分析预后的不足，同时，进一步研究可能指导促醒性治疗的药物选择。

Clinical Study of Resurgent Treatment to PVS after Cranial Operation

Objective To evaluate the effect of medicine in treatment to persistent vegetable state(PVS) after cranial operation and investigate correlation between electroencephalogram reaction and result.Methods According to electroencephalogram reaction to naloxone,40 cases were divided into two groups,group A having obvious reaction,while group B no.All patients were given same treatment for two months.Results After two months treatment,9 patients inside A group came to conscious,3 patients inside B group came to conscious.Analysis to statistical data show that resurgent rate of group A was obviously higher than group B.Conclusions According to the patients' electroencephalogram reaction for naloxone,we can preliminarily evaluate treatment effect and to compensate insufficiency of traditional assessments.
