青少年体重指数与血清瘦素等生化指标之间的关系

摘    要:目的研究青少年肥胖程度与血清瘦素、胰岛素、血糖及血脂等的相关性,为成年期疾病的早期预防提供相关依据。方法随机选取83名中学生,其中肥胖学生47名,正常学生36名。测定血清瘦素、胰岛素、血糖及血脂等指标,计算胰岛素抵抗指数(HOMA-IR)。结果除血糖外,血清各项检测指标肥胖组均高于正常组(P值均〈0.05);83名中学生BMI和血清瘦素及其他各项指标均呈显著正相关(P值均〈0.05)。女生只有三酰甘油、胰岛素和胰岛素抵抗指数与瘦素水平显著正相关。结论BMI对血清瘦素水平影响最明显,对糖、脂代谢的影响相对较小;肥胖组存在明显的高瘦素血症(瘦素抵抗)、高胰岛素血症、胰岛素抵抗(IR)和轻微的脂代谢异常。

Abstract:Objective To investigate the relationship between BMI and serum leptin,insulin,serum glucose and lipid in adolescence,and to provide evidence for the early prevention of adulthood diseases.Methods Eighty-three students were chosen randomly,47 were obese and the other 36 were normal.Their fasting serum levels of leptin insulin,glucose,total cholesterol(TC),triglyceride(TG)and low density lipoprotein(LDL)were analyzed,and homeostasis model assessment of insulin resistance(HOMA-IR)were calculated.Results The indices in obese adolescents were higher than the normal except glucose(P<0.05).BMI and serum leptin were positively correlated with serum indices but glucose(P<0.05).In female obese students,only TG,insulin and HOMA-IR were positively correlated with serum leptin.Conclusion BMI influences serum leptin significantly and slightly influences glucose and lipid metabolism.High prevalence of hyperleptinemia,hyperinsulinism,insulin resistant(IR)and slightly abnormity in lipid metabolism.
