高龄颈动脉狭窄患者经皮腔内支架成形的相关研究

摘    要:目的回顾性分析颈动脉支架成形术治疗高龄(年龄≥75)颈动脉狭窄患者的安全性、有效性、围术期并发症情况。方法对15例高龄颈动脉狭窄患者经磁共振和数字减影血管造影(DSA)确诊后行经皮颈动脉支架和椎动脉支架血管成形手术。16处颈动脉病变共置入17枚支架,技术成功率为100%。结果术后随访6个月,术中及术后明确诊断颈动脉窦反应3例;未出现典型脑过度灌注综合征患者;未出现严重脑缺血事件;未出现消化道出血患者;2例术后诊断为胃肠功能紊乱。结论颈内动脉支架成形术治疗高龄颈动脉硬化狭窄疾病有良好的安全性和确切的疗效,但是高龄患者的病情复杂,为降低并发症,严格的术前准备、规范的术中操作、严密的围术期管理仍然非常重要。

Transluminal angioplasty and stenting for the aged patients of carotid artery stenosis

Abstract:Objective To evaluate the safety, effect and complications of carotid artery stenting in elderly cases ( age greater than 75 ) with stenosis of carotid artery. Methods After the diagnosis of carotid artety stenosis in 15 aged cases was confirmed by MRI and DSA, the procedure of percuteneous angioplasity and stenting of carotid artery were proformed. Seventeen stents were implanted succefully in 16 stenosis lesions of carotid artery. Results Following up for 6 months, carotid artery sinus reactions were obversed in 3 cases. No case was diagnosed definitely as hyperperfusion syndrome. No case was considered to be related with ischemic cerebral aretery events. There was no digestive hamorrhage occurred. Two cases was diagnosed gastrointestinal dysfunction after procedure. Conclusions Carotid artery stenting is safe effective therapy for aged patients with carotid artery stenosis. Due to the complex of aged patient' s condition, sufficient prepare, standard procedure, correct treatment is still important to decrease the incidence of CAS complications.
