老年退行性心脏瓣膜病患病率调查和对心脏重构影响的研究

目的研究老年退行性心脏瓣膜病（SDHVD）患病情况和对心脏重构的影响。方法对3896例老年人进行心脏彩色多普勒超声检查,其中男性2884例,女性1012例,年龄60~103岁,平均（78±13）岁;调查SDHVD的患病率,并对有或无SDHVD两组心脏重构的相关指标进行观察。结果 SDHVD患病率60~69岁为12.0%、70~79岁为32.6%、80~89岁为61.0%、≥90岁为95.4%,且男性均高于女性（P〈0.05）;SDHVD组右心室流出道、主动脉根部内径、左心室内径、右心室舒张期内径、空间隔厚度、左心室舒张期内径数值明显增加,与无SDHVD组比较,差异均有统计学意义（P〈0.05,P〈0.01）。结论 SDHVD随着年龄增长患病率增高,SDHVD对心脏重构影响明显,并对判断预后有重要意义。

Study of prevalence and effects of cardiac remodeling on senile degenerative heart valvular disease

Objective To discuss prevalence and cardiac remodeling on senile degenerative heart valvular disease（SDHVD）. Methods Three thousand eight hundred and ninty-six patients （2884 male, 1012 female） with age of 78_＋13 （60-103）were assessed by color Doppler ultrasound. They were divided into SDHVD group and without SDHVD group to study prevalence of SDHVD and effects of cardiac remodeling on all patients. Results The prevalence of SDHVD were 12.0% with age of 60-69,32.6% with age of 70-79,61.1% with age of 80-89,95.4% with age of greater than or equal to 90 years. There was a higher prevalence of SDHVD in male than female （P〈0.05）. The RVOT,AO,LA,RVDd,IVST,LVDd were Increased significantly in SDHVD patients,and there were significant differences between two groups （P〈0.05）. Conclusions The prevalence of SDHVD increases with increasing age. SDHVD has obvious effect on cardiac remodeling, and there is important significance in judging prognosis.
