摘    要:
目的 探讨经颅磁刺激膈肌运动诱发电位 (diaphragmaticmotorevokedpotential,DMEP)在健康人群中的电生理学特点。方法 分别经项部及经颅部磁刺激 3 1例 (共 61侧 )健康志愿者 ,并用皮肤表面电极记录各受检者的DMEP ,并分析其电生理学特点。结果 所有受检者在深吸气易化时 ,均可引出DMEP ;右侧颈潜伏期大于左侧 ,皮层潜伏期、颈潜伏期与身高呈正相关 ,中枢运动传导时间 (centralmotorconductiontime ,CMCT)不受侧别、性别、年龄、身高等因素影响 ;本组受检者左侧颈潜伏期为 ( 7.8± 0 .6)ms ,右侧颈潜伏期为 ( 8.3± 0 .6)ms ;合并不同侧别及性别后皮层潜伏期为 ( 12 .2± 1.0 )ms ,CMCT为 ( 4 .1± 0 .7)ms。结论 DMEP是评价与呼吸有关的皮质脊髓束功能的重要电生理学指标之一。

Abstract:
Objective To investigate the eletrophysiological features of diaphragmatic motor evoked potential(DMEP) elicited by transcranial magnetic stimulation in the healthy subjects. MethodsTranscervical and transcranial magnetic stimulation were applied, respectively, on 31 healthy subjects at both sides, the DMEP was recorded by use of surface electrode, and its electrophysiological features were analyzed. ResultsDMEP was obtained from all of the subjects with facilitation by deep inspiration. The right cervical latency was longer than the left one. Cortical and cervical latencies were positively correlated with the heights of subjects. There was no correlation between the central motor conduction time (CMCT) and such factors as the recording side, sex, age and height. The left cervical latency was 7.8±0.6 ms, and the right one was 8.3±0.6 ms. After combining the results of both sides and sexes, the cortical latency was 12.2±1.0 ms, the CMCT was 4.1±0.7 ms. ConclusionDMEP is one of the important parameters used to evaluate the corticospinal tract function as related to respiration.
